    Hi there,

1> We have used contact form7 for making contact form. Here is the link for same Contact form 7 link.
To display on specific page we have used widget visibility plugin by jetpack. You can see the documentation Here
Also you can see html snippets on theme bundle that comes when downloading theme from our site.[ lavish-pro-package-110\lavish 1.1.0\snippets\Form-elements].

Note: widget visibility comes along with jetpack so you just need to install jetpack plugin.@see video tutorial for jetpack

2> We have used page right sidebar areas with text widgets for content on right side.

3> For google map we have used https://wordpress.org/plugins/wp-google-maps/ plugin shortcode in banner wide widget position[Text widgets content area is used for placing shortcode ] with jetpack widget visibility plugin

Let us know if you get any problems on this.

    Thanks

    Hello there,
Have followed your recommendations yet the contact form isn’t delivering messages (Junk folder didn’t contain any either).

    Your Name (required)
    [text* your-name]

    Your Email (required)
    [email* your-email]

    Subject
    [text your-subject]

    Your Message
    [textarea your-message]

    [recaptcha]

    [submit “Send”]

Please can you indicate what could be going wrong?

    Thanks

    Hi there,

Please take help from Html snippets that comes along with your theme bundle. Go lavish-pro-package-110->avish 1.1.0 -> snippets-> Form-elements. Try keeping those id and class on form elements.

    Thanks
